If the applicant fails the examination, it is the applicant’s responsibility to retain the Checklist and the required documents that would have been given to the Examination Proctor if the Checklist stated that documents were required. The applicant can then go to the CDI’s Web site to reschedule the examination using the Online Examination Scheduling service. When the examination is rescheduled, the applicant is to take the Checklist and the required documents to the Examination Site. 7. What examination sites will accept the Checklist?
