What happens if I change employers but remain a Caremark plan participant?
If your employer changes but your prescription benefit manager is still Caremark, you will need to contact Caremark to update your member ID. You will be sent a new authorization form to complete so that your signature authorization matches your new member ID on file and you can continue to earn Caremark reward points.
