If an otherwise eligible patient is admitted to a hospital other than St. Luke’s as a direct result of an adverse outcome caused solely by an IFPM urogynecologist’s technical portion of the pelvic reconstructive surgery utilizing mesh performed at St. Luke’s, the patient is not eligible to participate in the limited guarantee.
