The student must contact the instructor for each course as soon as possible to negotiate timely completion of requirements. Contact the School of Health Student Services Center (800) 344-5484) office if you cannot reach the instructor electronically. If a course is not completed, the eligible student may negotiate an Incomplete Grade Agreement form with the instructor, and complete the course as agreed.
