What happens during the In-Person Authentication process?
You can upgrade your account at your VA Medical Center or Community Based Outpatient Clinic (CBOC). Contact your My HealtheVet Coordinator or Release of Information Clerk and provide: • a government-issued photo identification (a Veterans Identification Card (VIC) or a valid driver’s license is acceptable) • your signed VA Form, 10-5345a-MHV VA Staff will: • match your first name, last name, social security number, gender and date of birth with your official VA health record. This process safeguards your information. Now My HealtheVet data will always match your official VA medical record. If you need to change information in your personal profile, please contact your VA facility to update your VA record. Please note: When you make any updates or changes in your My HealtheVet profile (for example, your address or phone number), those changes are not sent to your VA health record.
