Participants sign in on Attendance Sheet. Complete the appropriate forms-Medicaid Eligibility, Flowchart, LD Criteria, and Extended Evaluation Form or Referral for Additional Evaluation (if necessary). At the conclusion of the discussion, make the appropriate recommendations. The goal of the Team, and the mandate of Massachusetts Department of Education, is the appropriate placement of students in the Least Restrictive Environment (LRE). If the student qualifies for services, the parents should get a draft of the IEP service delivery page to take home.
