A. A preliminary report will be issued during the investigation, and a conference with the investigators will be available. Following the conference, the AD will issue an “Order to Show Cause Re: Administrative Penalty” (which includes the final report). At that point you may either stipulate to the findings or answer within 30 days. If you do not stipulate, an administrative hearing will be conducted. Once the order is final, the claims administrator or URO is required to send a copy of the final report to the insurer, self-insured employer or TPA. Investigation results will also be posted on the DWC Web site.
