What forms of documentation are required/accepted?
All students who apply for the subsidy must submit a copy of their child’s birth certificate or Visa. The document must include the student’s name, the child’s name, and the child’s date of birth. We will only accept copies of this documentation, not the actual certificate. International students will also be required to submit additional documentation including a copy of thier passport, I-94, and I-20 or DS 2019 depending on Visa status. Affected students will be notified upon approval of their subsidy.
