What forms must be kept on file?
Forms OMR 101 and OMR 102 must be kept on file for six years following the removal of the party as an authorized party or designee. Forms OMR 104, OMR 105 and OMR 106 or 106A must be kept on file for six years after the party ceases to be a subject party (generally, when he or she is no longer an employee). The OMRDD determination letter and new arrest notice must be kept on file for one year after it is sent by the CBC unit and must be destroyed at that point. In accordance with the prescribed time frames all information must be destroyed in a confidential manner.
