What forms must be completed to get a refund of employee contributions or a deceased members contributions?

To receive a refund, a member must complete a PERS Form 5, Member Refund Application. A beneficiary of a deceased member should complete a PERS Form 5D, Beneficiary Refund Application, and attach a copy of the deceased members death certificate in order to obtain a refund of unused contributions. Both forms can be obtained by contacting PERS or downloading them from our Forms Library.
