To receive a refund, a member of a retirement plan administered by PERS must complete a PERS Form 5, Application for Refund of Accumulated Contributions to Member; or a PERS Form 5D, Application For Refund of Accumulated Contributions to Beneficiary of Deceased Member, and submit this form along with the deceased member’s death certificate.
