What forms does a managing authority need to use?
There are six forms that hospitals and care homes might need to use in different circumstances. These are standard forms, and their use without alteration will enable Managing Authorities and care homes to keep the records required as part of the MCA DoLS process. The forms are available in a variety of formats as follows: • the full complement of forms in one document (not recommended to download); • individual forms formatted in .pdf with background colour; • individual forms in .rtf (that can be completed using Microsoft Word) also with background colour; • individual forms in .rtf without background colour: this is so that they can be scanne
