What forms are required for the dissertation defense?
The student must submit the Dissertation Prospectus Hearing Approval Form which can be found in the Doctoral Guidelines. The dissertation committee form must be submitted to the doctoral office within six months of the dissertation defense. The committee must be approved by Rackham, and any changes to the committee must be documented with a new dissertation committee form.
