What forms are required and who should have copies?
An open league coach should register players as a team. The registration and medical release form can be printed out for each team member from the WCYBS homepage. These forms must be completed at the time of registration. This is the same traditional registration form used for recreational league. We suggest having three copied sets of the previously mentioned forms, as well as your proof of insurance, USSSA roster, and all players birth certificates: two for coaches and one with the team mom.
