A general slot machine employee must submit the following forms: DBPR PMW 3410 – Individual Slot Machine Occupational License Application DBPR PMW 3460 – Authorization for Release of Information Two forms of identification as required by Rule 14.010, Florida Administrative Code. Fingerprints (must be taken by livescan at the slot machine facility). Fingerprints are due upon initial licensure and every three years thereafter.
