The Employee Status Form and Student Appointment Form were replaced by the Staff Personnel Action Form (PAF), Faculty Personnel Action Form (PAF) and Student Personnel Action Form (PAF). These forms are available on-line in excel or MAC format, and they can be typed in, printed, and/or saved for future use.
