• The Affidavit of Support and Bank Statement are forms created by Goldey-Beacom College to be completed by your sponsor. These forms must be signed and notarized. We cannot accept photocopies. If you have more than one sponsor, each one must complete a separate set of forms. Please make sure you keep a second original copy for you to prove to the United States Consular Office that you have sufficient funds.
