What does the trustee have to submit?
Documents related to the trustee organization that need to be submitted with the completed application by the submission deadline are: • Part A – Organizational Overview – Completed by the trustee organization • Trustee’s most recent AUDITED financial statement • Trustee’s Letters Patent • Trustee’s constitution/by-laws • Trustee Agreement – Signed by both the trustee organization and the applying organization • Trustee Authorization – Last page of the AEHR- CPIP application must be signed by the trustee organization • Declaration of a Non- Discrimination Policy (signed and adopted by the trustee organization) Please refer to the requirements in the AEHR-CPIP application.
