Your out of pocket maximum is the maximum dollar amount that you are responsible for each plan year. The Out of Pocket Maximum does not include expenses for usual & customary cuts or deductibles. Please refer to your plan booklet or contact our Customer Service department with any questions regarding your maximum out of pocket expense. After you have met your plans out of pocket maximum, benefits are allowed at 100% for the remaining benefit year. Benefit year maximums are based and calculated on a calendar year, January through December.
