What does my process documentation need to include for element 2.1.2 – Strengths and Needs Assessments?
The strengths and needs assessment is a process developed by the service and completed prior to the care plan. It is an information gathering and assessment tool. To meet this element the process documentation must include usage of the referral form (and other information obtained prior to and following the placement) as part of the assessment. For further information refer to the Resource Paper – Standard 2.
