What does discharge planning entail?
Our goal is to work closely with families so that their children are able to return home as soon as possible. Discharge planning begins on admission and is a collaborative process between team members and parents. It involves assisting children and families in acquiring the resources needed to be home safely. It involves arranging for services such as medical care, therapy, and schooling, acquiring equipment and supplies as needed, and making connections with community agencies that can provide ongoing help.
