What does APC stand for?
APC stands for Adult Primary Care a revolutionary system of providing more individualized care in the outpatient setting. This model has been embraced by most of the Northern California Kaiser Permanente facilities. In San Francisco, the Department of Medicine is comprised of 5 APC modules, each taking a multidisciplinary approach to patient care. A module consists of: 6-8 internists, 2 nurse practitioners, 1 diabetes clinical nurse specialist, 1 health educator, 1 physical therapist, 1 behavioral medicine specialist, 1 nurse case manager (who follows complicated patients with chronic conditions, such as dementia and HIV), and medical assistants. In addition, 4 6 residents are assigned to each module for their continuity clinic experience. This allows residents to become integral members of these modules and to learn first hand the multidisciplinary approach to patient care.
