The cost per alias is $1/month ($12/year), payable annually in advance, using either PayPal (Online) or check, cashiers check or Money Order (Offline). The form provided allows making your request, defines the names which are restricted, and gives space for selecting up to three alternative names (in the event your first choice name is either restricted or not available).
