The CGA includes a thorough medical history and physical examination by a geriatric case manager, emphasizing acute versus chronic conditions and medication history. Also, will suggest referrals for consultations with other experts, such as a geriatrician (a doctor trained to focus on the common conditions and special needs of the elderly), social worker, neurologist, psychiatrist, cardiologist, gastroenterologist, rheumatologist, and/or a physiatrist (physician of rehabilitation medicine). A Sunflower GCM completes a functional assessment (ability to perform activities such as bathing, preparing meals and managing finances) and a mental status examination. They also examine how changes in health and function are affecting the individual and his or her family. Emotional well-being as well as specific service needs of the client will be evaluated. The GCM considers financial implications in making appropriate recommendations.
