1. Application for admission , with a 2×2 inch photograph __(application form can be downloaded from Here ) 2. Reference letters ( 2 for Dip.P.H., M.P.H.,and 3 for Ph.D.) 3. A copy of transcript record 4. A report of TOEFL test with the score of 450 or IELTS with the score of 5 for M.P.H. , __Dip.P.H. and a report of TOEFL test with the score of 500 or IELTS with the score of 5.5 __for Ph.D.This can be exempted if you could provide evidence of your previous degree __taught in English or score of CU-TEP test << more 5.
