What documentation is required to submit vestibular testing claims?
When providers receive Additional Development Requests (ADRs) they should submit all of the documentation requested on the ADR: • Physician orders; • Clinical documentation of diagnosis or symptoms to justify services; • Reports of audiology / services rendered; and • An itemized bill. • History of present illness and prior treatment; • Any other contributing diagnosis or factors; and • Documentation of vestibular rehab services.
