OSHA requires the following documentation regarding operator evaluations: 1910.178 (l) (6) The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. MH Equipment will provide your company with all the required documentation to show OSHA compliance. You will recieve copies of the sign in sheets complete with signatures of the attendees and the instructor, the written tests, and the operator evaluation results. Attendees will also receive a certificate of completion.
