Your Fund requires a completed, signed Census Enrollment form to be on file (available online) as well as a copy of your birth certificate, your spouses birth certificate and marriage license if applicable, Certificates of Live Birth for any dependent children as well as any information concerning other Health Care coverage for yourself or any member of your family.
