What documentation is required to get reimbursed for the Medicare Part B premiums since they are automatically deducted from my Social Security deposit each month?
The Social Security Administration issues a 1099 at the end of each calendar year. You may submit a copy of that document along with a claim form specifying the monthly premium and the period of time for which you are requesting reimbursement. Social Security may be able to generate a quarterly statement detailing the premium you have paid for Medicare Part B. If you request and receive such a statement, you may use this to file for reimbursement. The statement must be printed with the coverage period, amount paid and name of the covered person. You will be required to submit this information with a signed claim form.
