What documentation is required for DEERS Enrollment and ID card issuance?
A DD Form 1172 is required for ID card issuance and DEERS Enrollment (see question #4). For family members already enrolled in DEERS all that is required is the current military ID card and one other form of ID. If the military ID has been lost, you will need to provide another form of photo ID. Please see page 5 for the List of Acceptable documents from the Form I-9, Employment Eligibility Verification.
