• A Passport valid for travel to the United States with a validity date at least six months beyond the applicant’s intended period of stay in the United States. Old passports (whether or not they include U.S. visas, but especially if they include still-valid U.S. visas) should also be presented. • The confirmation page of completed online visa application form DS-160. Only the confirmation page will be accepted. Please, remember to fill out the form DS-160 at least 72 hours prior to your interview. • The original receipt from Citibank for payment of the application fee.
