The Board will accept a certified copy of your birth certificate provided it is issued by the appropriate city/county/state/federal certifying authority if you were born in the U.S. If you are an American citizen who was born abroad, we will accept a certified copy of a Certification of Birth (DS-1350) or a certified copy of the Consular Report of Birth Abroad (FS-240). We do not accept hospital birth certificates, photocopies, notarized photocopies, foreign birth certificates, or passports. Passports are not accepted as proof of meeting the Rule II(a)(5) citizenship requirement. Individuals who do not have their Citizenship/Naturalization documentation available need to contact U.S. Citizenship and Immigration Services.
