The certifying professional or advocate should complete Section 3 of the form, and there should be a letter from the person who diagnosed the student that clearly states the diagnosis, the date of diagnosis, and any additional supporting documentation not covered on the form. The GED Examiner at the adult education center will have to complete the rest of the form and send it to the State GED Testing office – the sooner, the better, so the student knows what accommodations will be applied during instruction and testing.
