Medicare requires a signed prescription from the ordering physician that includes a diagnosis to support the medical necessity of the overnight oximetry test (97462). The prescription must be faxed to VIRTUOX prior to the oximetry upload. You may also use the physician prescription provided by VIRTUOX. In order to bill Medicare or other insurance provider, VIRTUOX will also need the patient to sign its Assignment of Benefits (AOB) form. Once you enter the patient and physician information in the VIRTUOX application, an AOB pre-populated with that information will print. You will need to fax that form to VIRTUOX’s toll free fax line prior to upload.
