What do we do when a post-accident test is required after regular working hours and the proper chain-of-custody forms are not available?
Every effort must be made in advance to prepare for these type of situations to ensure that the necessary forms are available. Supervisors should therefore maintain a supply of these forms. However, in those cases where you must use forms provided by the medical facility, please notify your HQ HR Analyst as soon as possible, so that receipt of the test results can be coordinated.
