What do the numbers above each bar in Table No. 2 (above) mean?

The numbers above each bar indicate a score derived from the violations and the severity of each violation by that nursing home for that year. The bar at the far right hand side indicates the 4 year average of that same score for all nursing homes in the state in question. You can therefore compare how the nursing home you are considering compares with the average of all nursing homes in that state for the past four years. (Note: N/A under some bars means that the nursing home was not inspected that year).
