You’d need to give a call to your Office of Vital Statistics and find out what they need from you in order for you to receive a BC. For example, where we live, I call and inform them that I will be having a homebirth and will need a BC in the future for my child. The lady then sends me a Birth Certificate application that I will have to fill out, and in addition to sending this back to them after the baby is born, I need to also send a proof of pregnancy (statement from a doctor or midwife confirming that you were pregnant), a well-check up from the doctor stating baby was born live and well, a photo I.D., and a house bill (electric bill, etc.) as proof of address. After I get all of these together, I send it all in. Each state may differ slightly, some requiring more and others less.
