Enclose a copy of your entry form (or your online entry confirmation) with your submission on a labeled NTSC VHS videocassette copy or DVD (region 0 or 1) with Amaray case that meets criteria (see below) and the applicable entry fee. Be sure to sign the form. Do not send original materials.
