Brake fade is caused by the brake pads overheating. If this happens all the time and not just during hard braking, then the pads are glazed over. You can lightly sand them to remove the glazing, or just replace the pads. Now if you are referring to brake pedal being soft, then you have air in the system. Bleed your brakes.
