What constitutes a complete free & reduced price meal application?
PART 1 (Student Information) and PART 4 (Signature & Social Security Number) MUST be completed on every application. • PART 1- Complete information includes: • Student(s) names • School attended by each student in OKC district • Grade for each student • Birth date for each student • TANF, Supplemental Nutrition Assistance Program (SNAP), formerly the Food Stamp Program, FDPIR numbers, if applicable • PART 4- Complete information includes: • Signature of Adult Household Member • Social Security Number of Adult Household Member • If no SS# available write the word “NONE” or check box • Printed name of Adult Household Member • Home Telephone Number • Work Telephone Number • Address • Date If a TANF number or SNAP number is listed in PART 1, the application shall be approved for free meals if the parent has furnished all other required information in PART 4. • PART 3- Complete information includes: • Names of All Household Members • Gross Income for each household member and how often rece
