If you have syncing available, try resetting the database on the sync server by making sure that you have a client that is fully-synced, then moving aside the OmniFocus.ofocus file on your iDisk (the remote copy – not a locally-synced iDisk) or WebDAV server, then syncing again with that fully-synced client, to push all the data back. If that does not help, try performing a backup in OmniFocus on the Mac (after making sure that it is fully synced), then restoring from that backup, and then resetting the database as described above.
