What can I do if a key provider – for instance, a doctor or hospital – does not participate in ChoiceCare Network?

If you’ve searched the ChoiceCare Physician Finder Plus and can’t find a specific provider, you can nominate a doctor or hospital for addition to our network by completing the online Provider Nomination Form or Facility Nomination Form. Our contracting team will evaluate your request to determine whether our adding the nominated provider would impinge upon existing contractual relationships with in-network providers. If no conflict of interest exists, we will contact the nominated provider on your behalf and mail the provider a network participation agreement. ChoiceCare Network cannot guarantee that nominated providers will agree to participate in the network or that nominated providers will meet other network participation requirements – e.g., credentialing.
