What attachments are required to be included in the SICA application?
All applicants must include the following two attachments in their application: a completed Prevention System Logic Model and a completed Community Substance Abuse Prevention Planning Logic Model. Any applicant that is not a Tribal Council but is applying for funds for initiatives on tribal reservations must also include an approved Tribal Resolution authorizing the initiative as a third required attachment. No other attachments will be accepted or reviewed (e.g., annual reports, brochures, etc.) and will be stripped from the application prior to the review process.
