What are things to remember when coming for an Assessment?
• Most assessments will take between 1 – 2 hours but may take longer if the assessor is attempting to secure state funding for your treatment. Do not bring children with you. • Come 15-20 minutes before appointment time to complete initial paperwork. • Bring all medication and a current Medicaid or insurance card. Bring all paperwork related to a court order or arrest. If your Assessment is due to DWI, include blood alcohol (BAC) at the time of arrest (verified by breathalyzer print out), certified BAC copy or blood work results. If you do not have your documentation, you will need to obtain a copy of it from the County Clerk’s office. If your records are no longer available, you will need to obtain a written statement from the Clerk’s Office saying that the records are not available. • If you have been assessed elsewhere, you must have documentation from the prior assessor, including the date of your Assessment and the treatment or education recommendation.
