You can re-activate at any time using one of the following methods: Website – To re-activate through the website, log into your account. Once you have logged in, you may select the Prescriber Activation tab located on the top left side of your homepage. You are then required to read the list of statements that appear on the screen. After you have read the statements, you must attest to the statements. IVRS – Automated Phone System – To re-activate through the IVRS automated phone system, log into your account. Once you have logged in, listen to the menu choices and select the prompt to activate. The system will read the attestation/activation statements then request you attest by selecting the correct menu choice on your phone keypad. • Can prescribers edit their own (demographic) information? Yes. Prescribers and Designees can edit the following prescriber demographic information: • First and Last Name • Middle Initial • Specialty • Address • City, State and Zip Code • Phone Number •
