What are the steps to discontinue a Patient?
Directions for discontinuing patients that are Lost to Follow Up are as follows: • Log on to http://www.ipledgeprogram.com/ and go to the manage patients screen. • Select the appropriate patient from the patient list by highlighting his/her name. (You can search for LTFU patients by clicking on the status tab). • Enter any new pregnancy test results. (If not available, proceed; pregnancy test results may be entered while the patient is in Inactive status) • Select the option to discontinue patient towards the bottom of the screen. • Choose the reason for discontinuation. If you choose other you must enter a brief comment explaining the reason for discontinuation. • Once you have entered the reason for discontinuation, select discontinue patient. You will then be prompted to enter Date of Last Dose. Please enter the date of last dose (the day the patient last took medication and NOT the date of last prescription) in MMDDYYYY format. • Choose discontinue patient and you will notice that
