What are the steps to complete the Perkins promissory note and Student Loan Questionnaire?
The Perkins Promissory Note and Student Loan Questionnaire must be completed online before the loan can be disbursed. You will need to login to My Aid Status at https://www.finaid.ucsb.edu/AidStatus/index.asp and proceed to Tab 5. Once you accept the loan, you will be notified by email when the Student Loan Questionnaire and Master Promissory Note is available to complete.
