Detailed requirements and the New Student Immunization Form can be found online at http://scc.uchicago.edu. The form must be returned to the Student Care Center (SCC) – no exceptions will be made. Finally, undergraduates are strongly encouraged to consider immunization against Meningitis, Hepatitis B and Varicella Zoster (chicken pox). It is very important to submit immunization forms to the Student Care Center either before arriving on campus or on Opening Day at the SCC tent at the Family Resources Fair.
