Ahh, the fun of asking for recommendations. A couple of notes about it first: 1) Make sure you know the professor well. If you don’t have one you “know” well, you should have taken at least 2 courses with them. 2) Provide a copy of your transcript and your resume. Professors have so many students they can’t keep track of all their students grades’. As for the timeline, I would just tell them that you need it by December 1st. That should give them plenty of time. But if you haven’t heard back from them 1 week before then, call them to gently remind them – I had a friend who’s prof forgot about her letter! This will give you some lee way if necessary. Give your professor the option for the paper or online process (make sure you provide pre-printed paper forms) – it shouldn’t matter to you which way they choose because letters of recommendation are supposed to be private and sealed so you don’t get to read them anyways. Most letters are actually a series of boxes for the prof to checkmark
