1. Have a formalized relationship with the College of Pharmacy through the submission and approval of documents for an appointment in the College. 2. Have a comprehensive, patient-focused professional practice. 3. Be a licensed or registered health care practitioner in good standing with his/her respective state board of practice and/or be certified by his/her board if there is such a process in his/her profession. 4.
